How to Fix Marionette Lines in Seoul: Non-Surgical Options
Marionette lines run down from the corners of the mouth and can make you look sad or tired. Our physicians soften them by supporting the mouth corners, relaxing the muscle that pulls down and lifting the jowl beside them.

What causes marionette lines?
Marionette lines form when volume around the mouth corners is lost, the jowl above sags, and a small muscle (the DAO) pulls the corners down. Filler, a little Botox, D-Jamber threads and lower-face lifting can soften them without surgery.
As the lower face ages, the fat beside the chin shrinks while the jowl above it slides down. The skin folds where these two areas meet, creating a line from each mouth corner towards the jaw. Thinner skin and lower collagen let the crease set in deeper over time.
The depressor anguli oris (DAO) muscle pulls the mouth corners downward. When it is strong, the corners turn down even at rest, which makes the lines look worse. Treating marionette lines well usually combines support for the corners, relaxing that muscle and lifting the jowl.
Signs these treatments can help
- Lines from the mouth corners that stay at rest
- Mouth corners that turn down and look sad
- Early jowls beside the lines
- You want a fresher lower face without surgery
When to see a specialist first
- Sudden drooping of one side of the mouth: seek urgent medical care
- Very loose lower-face skin, where a surgical facelift may be more effective
- Active infection, cold sores or upcoming dental work around the mouth
Four reasons marionette lines appear
Volume loss
Fat beside the chin shrinks, leaving a hollow under the mouth corner.
Jowl descent
Tissue above the jaw slides down and folds over the line.
Strong DAO muscle
The muscle that pulls the corners down deepens the line.
Thinning skin
Less collagen lets creases set in and stay.

How we treat marionette lines at our clinic
- Assessment. The doctor checks the corners at rest and moving, the jowls and the chin.
- Plan. Corner support, muscle relaxation and lifting are combined as needed.
- Numbing. Numbing cream before injections or threads.
- Treatment. Filler, Botox or D-Jamber threads placed by your physician.
- Review. We check the result at 2 weeks by photo, when any Botox effect is complete.
What to expect
- Swelling or bruising for a few days
- Avoid wide mouth movements and dental work for a week after threads
- No massage of the area for a few days
- Botox softens the pull-down over 1–2 weeks
Possible side effects
Common effects are swelling, bruising and tenderness. Too much Botox near the mouth can briefly affect your smile, so we use small doses. Rare filler risks include lumps and vessel blockage; threads may cause temporary dimpling.
