What causes under eye bulging?
Under eye bulging is usually caused by fat around the eyeball pushing forward through a weakened lower-lid support layer, often with a groove below it. Non-surgical treatment can blend the groove and firm the skin so the bulge looks smaller, but only lower eyelid surgery removes or repositions the fat.
Behind the lower eyelid sit three small fat pads that cushion the eye. A thin membrane called the orbital septum holds them in place. With age, and earlier in people with a family tendency, this membrane loosens and the fat pushes forward into a visible bulge. The skin and muscle over it also thin, and the cheek below can lose volume, which deepens the groove under the bulge.
The shadow cast between the bulge and the groove is what makes the eyes look tired. Non-surgical care works on the contour: filling the groove carefully, thickening thin skin and tightening it so the transition looks smoother. It cannot shrink herniated fat. For a large, firm bulge, lower blepharoplasty by an oculoplastic or plastic surgeon is the definitive treatment, and we will say so at consultation.

How we treat under eye bulging at our clinic
- Doctor consultation. We look at the eye straight on and in profile to judge bulge size, groove depth and skin quality.
- Honest assessment. If the bulge is large, we explain that surgery elsewhere will give the clearest result.
- Blending the groove. For suitable patients, a small, conservative amount of filler softens the hollow below the bulge.
- Skin and tightening. Eye Rejuran, Re2O Fine or Eye Thermage thicken and firm the lower-lid skin.
- Follow-up. We review the result after swelling settles and keep in touch on Instagram.
Aftercare
- Sleep slightly elevated for the first night or two
- Avoid rubbing the eyes and heavy eye make-up for a day
- Avoid alcohol and salty food for a few days to limit swelling
- Use daily sunscreen and sunglasses outdoors
Possible side effects
Bruising and swelling are common for a few days after under-eye injections. Too much filler near a fat bulge can make it look larger or cause a bluish tint, which is why we use small amounts or none. Rare risks such as vascular complications are explained at consultation.

Frequently asked questions
Can under eye bulging be fixed without surgery?
Mild bulging can look noticeably softer when the groove below is blended and the skin is firmed. The fat itself stays, so a large bulge will still need surgery for a clear result.
Is filler safe for under eye bulges?
In small amounts and in the right patient, yes. Filler goes into the groove, not the bulge. Too much can worsen puffiness, so our doctors are deliberately conservative.
How much does treatment cost?

Do you do lower eyelid surgery?

Is there downtime?
Usually 1–3 days of mild swelling, and sometimes a bruise that make-up can cover. Most patients return to normal activities the same day.
