The short answer

What causes full lower cheeks and buccal fat?

Buccal fat is a deep pad of fat inside the cheek that can make the lower face look round. Non-surgical treatments cannot remove the buccal fat pad itself; they reduce surface fat, tighten skin and slim a bulky jaw muscle, which often explains much of the fullness. Removing the pad itself requires surgery.

The buccal fat pad sits deep in the cheek, between the cheek muscles and in front of the jaw muscle. Its size is largely genetic, which is why some slim people still have a rounded lower face. It tends to shrink naturally with age, so a full face at 25 may look leaner at 45. Fullness in the same area can also come from fat just under the skin, a large masseter muscle, or early sagging of the jowls.

Because these causes look similar, the first step is working out which one you have. Energy devices such as ONDA and fat-dissolving injections act on fat closer to the surface, not on the deep buccal pad. Botox slims an enlarged jaw muscle, and lifting devices firm loose tissue. Surgical buccal fat removal is performed through the inside of the mouth by a plastic or oral and maxillofacial surgeon, and we do not offer it.

Signs these treatments can help

  • Soft fullness in the lower cheeks and along the jawline you can pinch
  • A square or wide jaw that bulges when you clench
  • Early heaviness or mild jowls that blur the jawline
  • You prefer gradual change without surgery or general anaesthesia

When to see a specialist first

  • You want the deep buccal fat pad removed: this is a surgical procedure, best discussed with a board-certified plastic or oral and maxillofacial surgeon
  • Facial swelling that is new, one-sided or painful, which can be salivary gland, dental or medical in origin and needs checking first
  • A naturally slim face where removing volume may cause hollowing later; see sunken cheeks
Causes

Four reasons the lower cheeks look full

1

Deep buccal fat pad

A genetic fat pad inside the cheek; only surgery removes it.

2

Surface fat

Subcutaneous fat over the cheeks and jaw that devices can reduce.

3

Enlarged masseter

A strong jaw muscle widens the lower face and can be relaxed.

4

Early sagging

Loosening tissue drops toward the jaw and adds heaviness.

How we treat full lower cheeks at our clinic

  1. Doctor consultation. We examine the cheeks at rest and when clenching, and pinch-test to separate surface fat, muscle and sagging.
  2. Honest recommendation. If the deep buccal pad is the main cause, we tell you that surgery elsewhere is the relevant option.
  3. Surface fat and laxity. ONDA, or a Titanium package with a fat-dissolving booster, targets fat and tightens skin.
  4. Jaw muscle. If the masseter is enlarged, Botox can slim it gradually over several weeks.
  5. Review. We review progress after the course and adjust, with follow-up on Instagram.

Aftercare

  • Mild warmth, redness or tenderness usually settles within a day or two
  • Avoid saunas and intense heat for a few days
  • Keep a stable weight; fat reduction works best alongside it
  • Report lumps, numbness or asymmetry that persists

Possible side effects

Energy devices can cause temporary redness, swelling and tenderness. Fat-dissolving injections may cause swelling and bruising for several days. Over-slimming a naturally lean face can make it look tired, so our doctors aim for modest, balanced change.

Frequently asked questions

Can buccal fat be removed without surgery?

No. The buccal fat pad sits too deep for devices or injections to remove. Non-surgical treatments reduce surface fat, slim the jaw muscle and tighten skin, which often improves a full lower face.

Do you perform buccal fat removal surgery?

No. Our clinic is non-surgical. If the deep fat pad is clearly the main cause, we will tell you honestly and suggest you consult a board-certified plastic or oral and maxillofacial surgeon.

How much does non-surgical lower-face contouring cost?

ONDA 80kJ starts at ₩650,000 per session, and a 3-session package with a bonus session is ₩1,950,000, VAT excluded. See ONDA prices.

How many sessions do I need?

Most patients do 3–4 ONDA sessions about a month apart. Fat reduction develops gradually over several weeks after each session.

Is buccal fat removal a good idea long term?

It depends on your face. Because buccal fat naturally shrinks with age, removing it in a lean face can lead to hollow cheeks later. A surgeon can advise on your anatomy.

Can I do this on a short trip to Seoul?

Yes. ONDA and jaw Botox have little downtime, so you can travel the same or next day. The course can continue over later visits.
